To purchase one or more ModelRisk licenses, download and complete the PDF order form and return to us by email or fax.

Download the PDF purchase order form here. You will need Adobe Acrobat Reader 7 or higher to open and fill in the form.
2. Fill in the form Complete the PDF form as accurately and completely as possible.
2.1. Personal data
Ordered by - please fill in your billing data here - all fields except "Fax" are mandatory.
Deliver to - This is the address to which we will send your backup copy of ModelRisk.
VAT # - If you are from a company located in the EU you can provide the company's VAT number to be VAT exempt.
Belgian customers, and customers within the EU who do not provide a valid VAT number are subject to 21% VAT.
Other - You can provide an internal order reference and any additional comments in the designated fields.
2.2. Items to order
In the middle section of the form you can list the items to purchase, as described on our pricing page. Possible items include:
ModelRisk - Professional License
ModelRisk - Student License
ModelRisk - Academic License
You can also fill in a percentage discount (see the pricing page for volume discounts) for ordered items.
If your discount corresponds to a quote or discount coupon you were given by a sales representative of us, please indicate the coupon or quote number.
For example, say you want to purchase 3 copies of ModelRisk professional (unlimited) license and you wish to pay in USD. For every order of 2-5 items we give a volume discount of 5 %. You would then fill in the following items:
Item description: "ModelRisk - professional license"
Quantity: 3
Discount (%): 5
Unit price: 4,950
Currency: USD

Note that we charge NO additional shipping or handling fees.
2.3 Payment method
Check or wired transfer - if you select this method, we will send you an invoice, including further payment instructions. Once we received payment you will be sent your license key, password, and ModelRisk backup copy (see Delivery below).
Credit card - we allow you to pay by credit card. For fast processing you can provide your credit card details directly on the form, or leave these fields open and we will get back to you to collect your details.
3. Return the completed order form
Return the completed order form to us by fax or e-mail, using the buttons on top of the form:
- use the Print form button to print the filled form, and send it by fax to the number:

- use the Submit by Email button to submit the form by e-mail.
4. Confirmation
Once we have successfully received your order and verified that your data are correct, we will send you a confirmation e-mail with further details on how to complete your order, including an invoice. If you provided your credit card data and we successfully processed it, the invoice will indicate zero due.
5. Delivery
Once we have received payment, we will send the license key and password for activating ModelRisk to your e-mail address, and to your delivery address by post (including a back-up copy of ModelRisk).
